POST-EMPLOYMENT REQUIREMENTS:

Prevention/Neglect; Delegated Nursing Training and/or Personal Care of Individuals; First Aid/CPR; Orientation; Related Position Required
Training
RESPONSIBILITIES/JOB DUTIES:
Under the occasional supervision of the Adult Services Coordinator, and in cooperation with other team members, provides support services
and training to assigned individuals as per the individual plan. Authors goals, supports and services for day habilitation services. Initiates
documents such as individual assessments and team member input, facilitates the meetings, interprets team input, creates the working
document, ensures recommendations are met, monitoring vocational and habilitation activities, and evaluates the outcomes. Authors,
maintains, monitors, and evaluates programs; provides input for and monitors behavior support plans that are in place; travels to individuals'
home to pick up for community-based programs as required; plans, organizes and implements club/community integration activities as per
individuals’ preferences, e.g., visit a variety of businesses, use a variety of transportation vehicles. Educates community on services provided
and conducts tours of facilities. Completes/collects necessary documents for intake determination and for the official day services file.
Completes electronic records required to monitor, develop goals, and provides input for IP’s for individuals served; identifies trends and
patterns of incidents of concern; develops appropriate responses and tracks those responses to incidents of concern; reviews monthly ICF/MR
billings for accuracy, notifying management of needs to modify contracts. Completes and modifies required forms, ensuring all team members
and support staff are notified about changes in programming, needs or other items about the individual. Maintains a safe environment through
training, monitoring and supervision; monitors and safeguards individuals’ rights; plays an integral part in the UI/MUI process; reports all
allegations and suspicions of abuse and neglect to the appropriate authorities as well as notifications to parent, guardians and appropriate team
members; formulates plans, programs and guidelines to maintain health and safety of individuals served. Attends and may conduct meetings
and inservice training sessions as required; serves as a resource and referral person to individuals and parents. Assists in monitoring
individuals in their work areas and regularly assists individuals between work areas and busses/vans. Performs other related duties as required,
to include but not limited to: serving on committees and/or serving as a trainer, lifting, carrying and moving individuals.
COMPENSATION: Starts at $34,523.68 - $41,428.42 (depending on level of degree and experience)
