Summary: Under supervision, provides basic nursing assistance and assists residents with daily living activities.

Essential Functions:
  • Answers signal lights and bells to determine resident needs.
  • Assists residents with grooming, oral hygiene, bathing, hair, and incontinence care.
  • Obtains food trays and assists residents with feeding.
  • Assists residents with range of motion exercises, and movement to wheelchair or activity areas.
  • Assists resident with turning and positioning in bed
  • Assists resident with ambulation for short distance in facility.
  • Takes and records temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output, as directed.

  • Documentation on flow sheets as directed.

REQUIRED SKILLS:

DESCRIPTION/RESPONSIBILITIES:
Prepare exciting meals that meet the dietary guidelines. Your "customers" (our residents) are sure to appreciate your special efforts in making their food enticing while making sure it's the nutritional, healthful food they need.

Responsibilities
  • Read menus, recipes, and review all necessary steps in meal preparation with the Dietary Services Manager
  • Prepare all necessary menu items, determine quantity of food needed and follow recipes
  • Serve meals using portions listed on menu; use proper food preparation and handling techniques
  • Prepare substitute items to meet resident's individual needs
  • Cover, date and label all leftovers and store properly
  • Follow established security routine; use proper infection control techniques and protective equipment; use proper safety techniques
  • Receive and check all deliveries against invoices issued by the Dietary Services Manager
  • Supervise dietary personnel in the absence of the Dietary Services Manager
  • Perform other duties as assigned
REQUIRED SKILLS:
  • The Cook must be knowledgeable about safe food preparation and handling techniques, nutritional standards/applications and be skilled in bulk food preparation. Should be accurate, concise and detail-oriented and able to work cooperatively as a member of a team. Effective communication with residents, their families and at all levels of the organization is important.
  • Education at the tenth grade level or above is desirable with six months of experience in large quantity food preparation experience with therapeutic diet preparation is preferred. Must be able to read, speak and understand the English language and have basic math skills. Should understand the principles of growth and development for the entire life span and be able to provide appropriate care for adolescent, adult and geriatric residents.

About the Opportunity:
The Area Director of Admissions Mgmt is responsible for leading a collaborative marketing and sales approach through out all of the Kindred facilities that are located in the assigned market area. With respect to oversight of Area Director of Sales, strategy for revenue growth, daily and ongoing management of the sales personnel, management of the collaborative marketing budget, directing sales, marketing and admissions initiatives, policies and protocols.

  • 10 years in Healthcare Sales Management with responsibility for multiple facility locations and sales teams. Experience in leading, training and driving the referral through admissions sales process and growing new business.
Essential Functions:
  • Join all sales and admissions professionals from all Kindred Divisions together as a cohesive marketing and sales team for the Collaborative Market. This team will be responsible for promoting all services and programs offered throughout Kindred Healthcare in the specified Market, generating referrals and converting those to admissions in one of the Kindred facilities
  • Oversee the assignment of Territories by Referral Source to each Clinical Liaison.
  • Oversee the tracking system that will pull all admissions and referral numbers for all divisions together to create one focused report. Analyze these numbers and distribute to the appropriate leadership.
  • Support the sales and management tools as outlined in the Kindred POP Sales Training. Insure ongoing utilization of the tools.
  • Introduce a Sales Site Visit Reporting tool that includes proper utilization of tracking tools, sales call shadowing, relationship map utilization, potential admissions study, account management system.
  • Oversee the Assessment/Screening process by insuring that the collaborative assessment process is followed. Introduce the Collaborative Assessment process by following the guidelines, processes and procedures as required by all Kindred facilities.
  • Track the success of referrals within the Kindred system from one level to another and work with Case Management to continually improve the promotion of our system within our system .
  • Organize continued training on the programs and services offered at each level of care throughout Kindred.
  • Identify Program Development Opportunities throughout the Collaborative Market
  • Utilize a Tracking Program to identify trends and opportunities throughout the division.
  • Oversee the execution of annual plans, quarterly plans and weekly sales and marketing meetings are conducted for each facility.
  • Review all 90-Day Action Plans for those Facilities identified as focused facilities.
  • Provide a monthly overview of focused facilities in the Collaborative Market.
Knowledge/Skills/Abilities:

  • Basic computer skills with working knowledge of Microsoft Office, word-processing and spreadsheet software.

DESCRIPTION/RESPONSIBILITIES:

Summary: Through comprehensive assessment and evaluation, develops program of activity therapy from a holistic approach to meet the needs of a diverse resident population.

Essential Functions:
  • Performs comprehensive assessment for each resident to determine level of abilities combined with past and current interests.
  • Based on findings develops an individualized program of activity pursuits that are meaningful to the resident.

  • Assists in evaluating efficacy of interventions for nursing and physician action.
  • Develops a variety of activity "tracks" to appeal to a heterogeneous resident population, including assorted lifestyle desires (such as individuals who prefer self-initiated activities), cultural diversity and age-appropriate choices.
  • Utilizes the environment to provide both comfort and stimulation to the resident population.
  • Assists residents and staff in orientation to time and place by strategic use of center-wide bulletin boards and appropriate holiday decorations or reminders.
  • Arranges for speakers, presenters, advocates and clergy to interact with individuals and groups of residents.
  • Supports residents' needs and desires through 1:1 interaction, small group activity and large group meetings.
  • Organizes resident activities into a clear format (calendars and other communication tools) as a resource residents and staff can refer to.
  • Assists residents and guests to activities with the help of direct care staff
  • Arranges and coordinates off-site activity trips, when available or appropriate.
  • Provides a venue for resident activism through the resident council process; communicates expressed desires and concerns of those residents to Executive Director and appropriate department heads.

  • Collaborates with Rehabilitation Department to arrange activities that promote functional independence (Example: Provides range of motion activity plan with and for restorative nursing services).

  • Through in-service training, educates all staff members on approaches and models.
  • Collaborates with social services personnel in developing intervention strategies for residents with evident psychosocial concerns
  • Consults with Interdisciplinary Team on residents who may have behavioral disturbance or symptoms of a mood disorder, making recommendations based on observed and documented interests and needs.
  • Ensures all resident activity documentation is complete, and reflects analysis of effectiveness and reaches a conclusion on how to proceed with plan of care.
  • Utilizes Quality Indicators/Quality Measures Report to evaluate program efficacy for individuals and overall performance; makes program changes when needed.

DESCRIPTION/RESPONSIBILITIES:

REQUIRED SKILLS:

Must have knowledge of billing and collection practices/techniques and be skilled in the use of computers, particularly the Microsoft Office suite of applications. Should be accurate, concise and detail-oriented with the ability to organize and prioritize to meet deadlines. Flexibility about work hours and skill in directing and motivating staff are important. Also needed is the ability to communicate effectively with residents, their families and at all levels of the organization. About the Opportunity
Resident Activity Assistant organizes and participates in person-specific activity programming, including one to one interventions, small group facilitation and large group presentations and assists the resident activity director as needed.

REQUIRED SKILLS:
Education: Bachelors in Recreational Therapy or Occupational Therapy or Bachelors degree in relevant human service field, 20 hrs CEUs within 1 year of hire or state specific training to be initiated at first offering or Certification as Occupational Therapy Assistant or Associate degree in human service field, 20 hrs CEUs within 1 year of hire or NCCAP Certified as Activity Assistant Certified High School Diploma or GED AND minimum of 6 college credits*(as required by NCCAP) or State-specific certification as an Activity Assistant

Licenses/Certification: Certified Recreation Therapist, Certified Occupational Therapy Assistant or Activity Assistant Certified (NCCAP or state-specific) preferred

Experience: Prefer minimum of 500 hours of experience in activity programming in any setting prior to hire.
For high school/GED educated applicants, college courses must include one English course in composition, report or technical writing, and one course in areas of Art/Recreation Programming, Sciences, or Management with a passing grade.
