ADAPT FOR MEN (Job Code: 325-106)
Position: ASM A-1 (01020)

Company: Talbert House

This is a part time position working 24 hours/week, second shift, Monday through Friday and weekends. Requires High School diploma or GED equivalent. Prefer one year experience in a residential/security environment monitoring the activities of residents/others. A new hire must complete an internal training and become a certified drug test collector within 30 days of hire. Must have Microsoft Word computer skills to read and send emails on a regular basis as well as complete time sheets.

Job Overview: Monitor the activities of the unit and provide accurate and timely documentation as needed. Will provide awake and alert coverage, complete intake paperwork on incoming residents, collect urine screens, conducts random searches of the facility and any other duties as assigned. The successful candidate will serve as a positive role model at all times.

REGISTERED NURSE

ADULT CASE MANAGEMENT-COLLGE HILL (Job Code: 4001-81)

Position: RNS P-2 (300577)

Company: Centerpoint Health

This is a full-time, first shift position. Work days are Monday through Friday. Requires an Associates degree in Nursing, RN license, with one (1) year of relative experience in Medical/Surgical, LTC. Preferred candidate will have at least one (1) year of Mental Health experience. Microsoft Office computer skills are required. During the first eight weeks of employment, new hire must complete an internal training and pass a competency test on electronic clinical documentation software (CATT).

Other minimum requirements: Active RN license in the state of Ohio with no Ohio Board of Nursing disciplinary action against the applicant.

Job Overview: Administer medications/injections and provide mental health assessments and other related duties for a population of mentally ill adults. Additional responsibilities include med room management, nursing-centric trainings, and other duties. Requires a motivated individual who prefers a clinic environment, working with an inter-disciplinary team to best serve our clients.

WARREN COUNTY OUTPATIENT (Job Code: 306-17)

Position: CSP A-2 (02403)

Company: Talbert House

This is a full time position working first and second shifts. Work days are Monday through Friday. Program is located in Lebanon, OH (Warren County). Requires Bachelor's degree, RA (within 30 days of hire) and CDCA certification (within 90 days of hire). Two years experience working with AoD population and OCPS I preferred. A valid driver’s license and current auto insurance are also required for transport to and from work, as well as, in accordance with assigned job duties. Must have basic computer skills.

Job Overview: Perform AoD prevention services in the community and in schools. Will perform counseling and psychosocial intervention under supervision, act as a group facilitator using appropriate psychosocial interventions under supervision, interview and observe clients and prepare clinical reports and treatment plans under clinical supervision of an independent licensed counselor. Will also provide family counseling according to established principles and best practices, and may provide clinical direction and participate in providing in-service training to other treatment team members. The successful candidate will assure documentation is in compliance with contract regulatory and agency requirements and best clinical practices.

Physical Requirements: Infrequent to occasional standing and walking, frequent to constant sitting, rare climbing, infrequent balancing, stooping, kneeling, crouching, and crawling. There will be occasional to frequent reaching, and handling, rare feeling, tasting/smelling and use of color vision, occasional use of midrange and far visual acuity, field of vision and depth perception. Requires constant hearing and talking.

HEALTH-SOURCE OF OHIO LEASED SERVICES (Job Code: 7010-4)

Position: CSP P-2 (302073)

Company: Centerpoint Health

This is a part-time (24 hrs/wk), first shift position. Work days are Monday through Friday with some evening hours. Requires a Master's degree in Counseling, Psychology, Social Work or Marriage and Family with a LISW license or License in Psychology and two (2) years post degree experience in a Behavioral Health environment and provision of assessments/diagnosis, treatment planning, and counseling services. Preferred candidate will have at least three (3) years experience in provision of group, individual, or family therapy, and experience working in a medical office milieu and with demonstrated competency in Alcohol and other Drugs scope of service. A valid current driver's license and insurance is required to travel to rural locations for service delivery and/or trainings or meetings. Microsoft Office computer skills and data entry into electronic health records (EHR) system of leasing agency are required.

Job Overview: Provide Individual and Group treatment for Individuals and Families. Will perform assessments and treatment planning, using of best practice treatment pathways in an integrated medical clinic. Will be responsible for all clinical documentation and coordination of care with other providers. Position may also provide training to other agency providers.
