REGISTERED NURSE

DRIVER INTERVENTION PROGRAM (Job Code: 260-3)

Position: RN P-2 (00502)

Company: Talbert House

This on-call position is one evening/month from 4-10 p.m. This position requires an Associate’s degree, Ohio RN license, and one year of experience with signs and symptoms of substance abuse. The preferred candidate will have four years of hospital, residential or AoD treatment program experience.

Job Overview: To have the ability to quantify the presence of a life-threatening withdrawal, withdrawal seizures or delirium tremens; any of which can lead to death. RN to train the staff on what to look for if symptoms appear during the program. Complete the intake questionnaire with the clients and perform mental status exam relevant to signs and symptoms of drug withdrawal. Take clients blood pressure, check pulse and go over medication rules and requirements. This is to assist in sorting out individuals with high blood pressure and not properly medicated verses possible drug withdrawal. Complete all documentation.


HEALTH-SOURCE OF OHIO LEASED SERVICES (Job Code: 7010-4)

Position: CSP P-2 (302073)

Company: Centerpoint Health

This is a part-time (24 hrs/wk), first shift position. Work days are Monday through Friday with some evening hours. Requires a Master's degree in Counseling, Psychology, Social Work or Marriage and Family with a LISW license or License in Psychology and two (2) years post degree experience in a Behavioral Health environment and provision of assessments/diagnosis, treatment planning, and counseling services. Preferred candidate will have at least three (3) years experience in provision of group, individual, or family therapy, and experience working in a medical office milieu and with demonstrated competency in Alcohol and other Drugs scope of service. A valid current driver's license and insurance is required to travel to rural locations for service delivery and/or trainings or meetings. Microsoft Office computer skills and data entry into electronic health records (EHR) system of leasing agency are required.

Job Overview: Provide Individual and Group treatment for Individuals and Families. Will perform assessments and treatment planning, using of best practice treatment pathways in an integrated medical clinic. Will be responsible for all clinical documentation and coordination of care with other providers. Position may also provide training to other agency providers.


PROJECT PASS (Job Code: 460-73)

Position: CSP P-2 (02365)

Company: Talbert House

This position is a modified full time position (employment is based on 42 weeks per year/1664 hours and follows a school schedule). Hours are first shift, Monday through Friday. Requires Master's degree in Social Work plus LISW licensure with supervisorial role in 90 days. Preferred candidate will have at least one (1) year school-based mental health experience. Also requires computer skill in Microsoft Office. Must be able to climb stairs and work in fast paced school environment. A valid driver's license and current auto insurance are also required for transporting clients to appointments and/or social activities. During the first eight weeks of employment, new hire must complete an internal training and pass a competency test on electronic clinical documentation software (CATT).

Job Overview: Provide school-based counseling and case management to youth population (carrying a caseload of 15-18 clients), and coordinate treatment with school, community, and family. Will also complete diagnostic assessments, engage in home visitation and treatment planning, provide effective clinical supervision to 1-6 social workers seeking independent licensure, and accurately assess clinical training and supervision needs. The successful candidate will enable staff to use creative intervention to assist in meeting client's needs, provide clinical direction, and participate in providing in-service training to other treatment team members.


(NEW!) CHILD AND FAMILY CASE MANAGEMENT WESTERN HILLS (Job Code: 3005-4)

Position: CSM A-3 (304064)

Company: Centerpoint Health

This is a full time, first shift position working Monday through Friday. Requires equivalent of Bachelor's degree and work with children and family providing community case management services. Course work in Child Development, Mental Health, or Criminal Justice with a Mental Health Certificate only; one year case management experience or service provider experience in child mental health and child development; or license in social work or counseling is preferred. Must have basic computer skills. New hire must be able to meet the Agency's requirements for documentation and utilization. A valid driver’s license and current auto insurance are required. During the first 8 weeks of employment, new hire must complete training and pass competency test on electronic clinical documentation software (CATT).


Job Overview: Incumbent will perform all duties associated with CPST community case management, i.e., work with a caseload of clients, provide services specific to level of care established, and other duties associated with monitoring, referral, linkage, advocacy, education, etc. Provide support to clients and families, assist clients with daily living skills, provide referrals within the system when necessary. Documentation of interventions in electronic documentation system. Will work in home, school, and agency settings.

Physical Requireme New hire must be able to life and carry items up to 10 pounds and other objects nts:for clients when they are not able to. New hire's must be able to climb three flights of stairs in a building that does not have an elevator. New hire must be able to have their own vehicle, to use for assisting clients out into the community and meet Agency requirements for driver’s license. New hire must be able to meet the Agency's requirements for documentation and utilization.
